Fill out one Client Intake & Waiver for yourself and one Pet Profile for EACH PET individually. Please review ALL policies!

This form must be filled out for each customer/owner. Please fill out a separate form (below) for each of your pet’s profiles.

I certify that I am the legal owner of the pet(s) I am sending to the care of Pokk's Pets; that my pet(s) is currently and properly licensed; that to my knowledge my pet(s) has not been exposed to kennel cough, distemper, rabies or paravirus within the past 30 days.

I have read all of Pokk's Pets policies. I agree to pay any and all fees associated with the care of my pet(s), including but not limited to fees for unfixed dogs, property damage and excessive accidents.

I understand that pick up/drop off & care times need to be determined prior to the service dates. If for some reason I am unable to return for my pet(s), I will make additional arrangements for their care. If my pet(s) are not picked up 3 days after the service term has ended, they will be considered surrendered to Pokk's Pets and they may be rehomed or given to a shelter. If this happens, I am still fully responsible for paying all service fees and expenses incurred for the care of my dog.

I have disclosed to Pokk’s Pets all known risks, dangers and medical conditions associated with my pet. I understand that I am solely liable for medical care expenses and damages that result from injuries caused by my pet, or that I will resolve the matter directly with the harmed person or their harmed pet based on applicable laws. I understand that Pokk’s Pets may refuse service to my pet for any reason.

I agree that there are inherent risks in sending my pet to any dog activity center, such as illness or injury, and I accept these risks because they are outweighed by the benefits. Pokk’s Pets will not be liable for any illness or injury that may occur to my pet.

I authorize Pokk’s Pets to take actions they deem necessary to ensure health & safety of my pet including action to resolve any medical problems that may arise while my pet is in their care. I agree to assume full financial responsibility for all expenses incurred as a result of their actions. Under no circumstances will Pokk’s Pets be liable for damages & I waive all claims or actions against Pokk’s Pets relating to care, control, health & safety of my pet arising while in their care.
